Hold the body of the lighter between your curled index and middle fingers. Curl your thumb and ring finger [or pinkie, whatever works for you] up, grab the top and open it by pulling it back over the body of the lighter. While your thumb and ring finger hold the top of the lighter, use your middle finger to strike the flint wheel.
